Effective August 12, 2026. This policy covers this website only — it does not describe how your medical records are handled once you become a patient.

This form is not for medical emergencies or protected health information. Do not submit medical record numbers, detailed health history, or other sensitive medical details through the website contact form. For urgent post-operative concerns, call the office directly at (909) 558-5500.

Scope of This Policy

This Privacy Policy explains how Department of Plastic Surgery and Dr. Subhas Gupta ("we," "us," "our") collect, use, and protect information submitted through the website located at plsurgeon.com (the "Site"). It applies only to information collected through the Site — such as the consultation request form — and does not govern the protected health information ("PHI") created or maintained once you become a patient of record. That information is governed by the Notice of Privacy Practices of Loma Linda University Health under HIPAA, available at lluh.org.

Information We Collect

When you submit the consultation request form on this Site, we collect:

  • Name (first and last)
  • Email address
  • Phone number (optional)
  • Area of aesthetic or reconstructive interest you select
  • Any message or details you choose to write in the free-text field

We do not use tracking pixels, advertising cookies, or third-party analytics scripts on this Site at this time. Loading the Site does connect your browser to Google Fonts (for typography) and, on the Contact page, to a Google Maps embed — both of which may receive your IP address as a standard part of how those third-party services are delivered. We do not control how Google processes that data; see Google's own privacy policy for details.

How We Use Your Information

Information submitted through the consultation form is used only to:

  • Respond to your inquiry and schedule or discuss a consultation
  • Route your request to the appropriate member of the office staff
  • Maintain a record of prospective-patient inquiries for the practice's own scheduling and follow-up

We do not sell, rent, or share your information with third parties for their own marketing purposes.

How Form Submissions Are Processed

This Site is hosted on Netlify, and the consultation form is processed using Netlify Forms, a service that receives your submission on our behalf and forwards it to the office by email or dashboard notification. Netlify acts as our data processor for this limited purpose and does not use your submission for its own purposes. You can review Netlify's own privacy policy at netlify.com/privacy.

Data Retention

We retain consultation form submissions for as long as reasonably necessary to respond to your inquiry and for the practice's internal scheduling records, after which they are deleted or archived in accordance with the practice's standard record-keeping practices. You may request deletion of a pre-consultation inquiry at any time by contacting us using the information below.

Your Rights

Depending on where you live, you may have the right to request access to, correction of, or deletion of personal information we hold about you as a website visitor. California residents have specific rights under the California Consumer Privacy Act (CCPA), including the right to request disclosure of the categories of personal information collected and the right to request deletion. We do not sell personal information as defined under the CCPA.
